THE FOLLOWING INFORMATION ONLY APPLIES TO AGENCIES NOT CURRENTLY RECEIVING MULTI-YEAR FUNDING FROM UNITED WAY:

To determine if your organization meets a few core requirements, your agency must start with a Qualifying Survey that will be available on October 31 and needs to be completed NO LATER THAN November 11, 2011. If your program successfully completes the qualifying survey you will be directed to the United Way application page to create an account if you don’t already have one, and will then have access to the online application. A Qualifying Survey needs to be completed for each program you are seeking funding for.

Note:  Before starting the Qualifying Survey you will need to be prepared to upload the following agency documents:

  • Audit (or internally prepared financial statements if not required by the State of Minnesota to complete and audit)
  • 2011 Board of Director's Roster (including full contact information)
  • 501(c)(3) eligibility document

ALERT: If you exit out of the Qualifying Survey prior to completion you will be required to take the survey over from the beginning.
